Just as at quick glance, a few things I noticed:

Expand the chords, waltzes usually have the full chords (it sounds like it in the original as well.)
Your missing a voice, it starts on Eb, by measure 7, you actually have 3 melodies.

I don't know about the G5 in measure 2..

And like FireArrow said, the offbeat chords are higher for the most part and complete triads usually.

About the first few measures, I always heard the instrument playing G Ab G F Eb D B as the melody and the instrument you used in your sheet as the countermelody. Does anyone else hear it this way? Or am I just weird?

I think measure 34 is a bit off in the left hand, but I haven't listened to the original song recently, so I may be wrong.

Oh, I thought that was an Eb. >_< But yeah your right about it being the melody.
