The wolves do not know each other, if they did they'd have a massive advantage with their numbers and their combined powers. By separating them it will force them to both act quickly and think twice about their wolfings, lest they take out one of their partners.

If Player A votes Player B, player C forges their vote onto player D, and then player E uses beheading, does Player B or D die?

Beheading trumps all votes and immediately ends the phase, whoever Player E chose will die.

Can he choose who dies even if it's not a KitB?  Because if the Forge happened before the Beheading, then there would be a different leader in votes.

Whoops, there I go again, jumping ahead of myself.

Forge just functions to change another players vote, beheading ends the phase and the current vote leader is lynched. The flow chart just indicates to players the priority of powers, which ones will supercede others.
