First off, it's actually not named Death Waltz;

It's a remix of a piano remix.

Second,

This guy simplified it himself and played it on the piano. He has also made a sheet of it, it's in the description of the video.

EDIT: His sheet isn't that great, lol (see page 2). So the sheet is our task to make..

This has links to a midi file and sheet music.  If someone can separate the parts somehow, then we could have all the parts we need.  Note, when I say we need to separate the parts, I mean some measures are completely black.
